Terms, Conditions & Treatment Consent

1. Ownership and Authorisation

I confirm that I am the legal owner of the pet named in this agreement, or that I have full legal authorisation from the owner to consent to physical therapy and rehabilitation services on their behalf.

I acknowledge that Comfort Vet is relying on this confirmation when providing treatment and that I have the authority to make all decisions regarding the pet’s care, including medical, therapeutic, and financial aspects.

2. Scope of Services

I understand that Comfort Vet provides veterinary physical therapy and rehabilitation services only, including manual therapy, laser therapy, extracorporeal shockwave therapy, acupuncture, exercise programs, and at-home rehabilitation exercises. These services are supplementary and do not replace the care of my regular veterinarian. Comfort Vet does not provide general veterinary diagnostics, medical management, or surgical care.

I acknowledge that the rehabilitation plan may be adjusted over time to suit my pet’s individual needs and my household circumstances. Follow-up consultations are often required to reassess progress, and each follow-up visit incurs an additional fee.

3. Acknowledgement of Outcomes and Liability

I understand that every pet responds differently to physical rehabilitation, and Comfort Vet cannot guarantee specific results or complete recovery.

I acknowledge that Comfort Vet cannot be held liable for any future veterinary costs, complications, or health issues that may arise, nor for any injury or damage my pet may cause to people, animals, or property.

4. Client Commitment to Treatment Plan

I agree to follow the rehabilitation recommendations provided by Comfort Vet to the best of my ability, and to encourage others involved in my pet’s care to do the same. I understand that failure to follow the prescribed plan may reduce the effectiveness of therapy, slow recovery, or limit improvement in mobility, comfort, and function.

5. Privacy, Confidentiality, and Media Consent

I understand that Comfort Vet will keep my personal information and my pet’s medical records confidential and will not share them with third parties without my permission, except where required by law.

I consent to Comfort Vet staff contacting my pet’s current or previous veterinarians or other relevant pet professionals as needed to collect medical history or discuss my pet’s care, unless I request otherwise. I also understand that anonymised case information may be used for teaching or research purposes, with no identifying details shared.

I consent to my pet being photographed or videoed during consultations or therapy sessions as part of their medical record. I agree not to record consultations, sessions, or conversations with Comfort Vet staff.

6. Use of Materials

I agree not to reproduce, distribute, or share any written, digital, or printed materials provided by Comfort Vet without prior written permission. If permission is granted, any copies will be unaltered, including branding and authorship, and used solely for my reference and the care of the named pet.

7. Acknowledgement of Consultation Advice

I acknowledge that any advice provided by Comfort Vet staff is tailored specifically for my pet’s rehabilitation and is not intended for use with any other animal or situation. This includes advice given during consultations, phone calls, emails, SMS, social media, reports, or any other official Comfort Vet correspondence.

8. Booking Instructions

Please contact Comfort Vet to arrange a booking tailored to your pet’s individual needs. A list of services with associated costs will be provided, and a suggested consultation date will be offered based on availability and your pet’s requirements.

Once you accept the proposed service and date, an invoice will be issued, and a deposit of 50% of the consultation fee is required, in accordance with our deposit policy. Payment of the deposit confirms your acceptance of Comfort Vet's Terms and Conditions.

Following the payment of the deposit, please ensure that all requested information—including medical history, environment details, activity notes, and video submissions—is provided within the specified timeframe to allow proper preparation for your pet’s consultation. Delays in submitting this information may result in reduced consultation effectiveness.

9. Initial Assessment and Pre-Appointment Preparation

  • To optimise the results of your pet’s initial assessment, preparation is required in advance. Please email the following information to Comfort Vet (info@comfortvet.com.au) at least 2 business days prior to your appointment. Alternatively you can use the "Patient Information Form" in the section below and send the video by email*

  • Please ensure your deposit has been paid in advance to avoid delays in reviewing your submitted information.

  • Medical and Surgical History

 Complete history, including current medications.

  • Activity History:

 Description of your pet’s previous (if applicable) and current activities, such  as walks, sports, beach trips, jumping in/out of the car, etc.

  • Current Concerns:

Description of any unusual observations, e.g., difficulty maintaining posture, excessive paw licking, changes in gait or movement, or other behavioural/physical concerns.

  • Environment Details:

Description or photos/videos of your pet’s environment, both indoors and outdoors (steps, ramps, flooring, beds, etc.).

  • Video Assessment:

A good-quality video (maximum 5 minutes) showing:

  1. Standing still

  2. Stand → sit → stand

  3. Stand → down → stand

  4. Walking and trotting (if possible), filmed from the side, towards and away from the camera

  5. Turning in a small circle in both directions

  6. A handler lifting a front limb to assess strength and posture

  • Recording tips:

* Film in a calm, familiar environment, using a neutral background and good lighting. Hold the camera steady at your dog’s eye level and capture multiple angles (front, sides, and rear). You may include short clips highlighting any concerning movements or behaviours.

** We recommend uploading the video to YouTube and sharing the private link via email.

10. Fees, Deposits, and Payment Policy

I agree to pay a deposit of 50% of the booked treatment at the time of booking my consultation. This deposit secures my appointment and covers administration costs, initial case preparation, and loss of revenue from last-minute cancellations.

The deposit is non-refundable but if a cancellation is made with sufficient notice, the deposit can be applied for future bookings (See cancellation and rescheduling policy)

I understand that Comfort Vet has a strict No Accounts policy. All fees must be paid by cash or credit card at the conclusion of each consultation.

I agree to submit all required documents—including medical history, diagnostic records, and relevant videos—by the requested due date (at least 48 hours before consultation). Failure to do so may reduce the clinician’s ability to prepare and may result in time being spent during the consultation to review or gather this information to the detriment of treatment time, for which Comfort Vet cannot be held responsible.

I understand that prices are subject to change without notice, except where a written estimate is provided. Estimates are valid until the recorded expiry date, or for three months if no expiry date is noted.

11. Cancellation and Rescheduling Policy

  • Notification Requirement:
    Clients must provide at least 48 hours notice to cancel or reschedule an appointment. Notifications may be made via phone, email, or SMS. The deposit will not be refundable but may be applied toward a rescheduled appointment if proper notice is provided according to this policy.

  • Late Cancellations/No-Shows:
    Cancellations made less than 48 hours before the scheduled appointment will incur a loss of the deposit (will not be applied towards a future appointment).

  • Rescheduling:
    Comfort Vet will make every effort to accommodate rescheduled appointments. Late cancellations or no-shows may limit availability of preferred time slots.

  • Exceptional Circumstances:
    Fees for late cancellations or no-shows may be waived at the discretion of Comfort Vet in cases of genuine emergency or illness.

  • Follow-up Appointments:
    Follow-up rehabilitation appointments are also subject to this policy. Any fees incurred due to late cancellation or no-show will not be applied to future sessions.

12. Communication and Follow-Up Consultations

  • I understand that Comfort Vet does not provide unlimited free advice or answers to questions outside of scheduled consultations. While staff may respond to minor queries at their discretion, all treatment recommendations and case management require formal follow-up consultations to ensure the safety and effectiveness of the rehabilitation plan.

  • Due to limited staffing and to make the best use of time during consultations, Comfort Vet recommends that all queries, questions, or requests for advice outside of the schedule appointment be submitted via email or text message rather than phone calls. A consultation may then be requested in accordance with the terms outlined in this agreement.

  • I acknowledge that follow-up consultations are essential and will be scheduled as recommended by the treating clinician. These follow-ups are essential for monitoring progress, adjusting the rehabilitation plan, and achieving the best possible outcomes for my pet. Failure to attend scheduled follow-ups may reduce the effectiveness of therapy and delay recovery.

13. Revisit Consultations

  • A revisit consultation is defined as an appointment for a pet that has been seen by Comfort Vet within the previous six months, or sooner if there have been significant changes in the pet’s condition or rehabilitation needs.

  • I understand that if my pet’s condition has changed substantially since the last visit, or if a full reassessment is required, a long consultation may be recommended to ensure that the rehabilitation plan is safe, effective, and tailored to the pet’s current needs.

14. Late Arrival Policy

  • If you anticipate being late to your appointment, please notify us in advance whenever possible. While we will make every effort to accommodate late arrivals, it may be necessary to shorten the session or reschedule your appointment to another available time slot.

  • For clients who regularly arrive late, Comfort Vet reserves the right to place restrictions on scheduling, which may include limiting appointments to off-peak days or times to ensure fair access for all clients.

Reason of presentation (include medical and surgical history and current concerns)

Please describe all the abnormal symptoms you are seeing with your pet (e.g. change in posture, not able to jump in the car, sleeping more/less etc)

How is the condition currently managed? Include medications (and intolerance if applicable), housing or activities modifications, orthosis, cart etc

Do you think your dog is in pain and if so, what signs of pain can you recognise?

What makes the signs seen worse? (weather, exercise, after a rest etc)

What are the current activities of your pet and what restrictions are currently present that you would like us to work on?

Describe your pet's environment (flooring, bedding, ramps, height of bowls, stairs etc)

Can your dog do basic obedience commands? (sit, down, shake, wait). Please comment if your dog has done fitness previously and knows further cues.

What are your goals with your pet and do you have a timeframe in mind?
